I confirm that by registering on behalf or as a company-sponsored participant:

1. the company is a direct employer of the participant
2. pays the participant's salary
3. provide statutory benefits such as CPF contribution, medical benefits, leave
4. the UEN provided is correct and accurate

I understand that my personal and company's information will be submitted to SkillsFuture Singapore Agency "SSG", and the UEN provided below will be used to verify the employer and employee relationship between the participant and company. 

I understand that my company must make payment for my course fees on my behalf.

I understand that I will be liable to SkillsFuture Singapore Agency "SSG" for any inaccurate information provided. Mandatory checks on CPF90 form may be made to verify the submitted information.

Do not proceed as 'company-sponsored' if the participant does not meet the criteria above.

*Please note that Directors and Sole-Proprietors who are not formally employed with CPF contributions are not considered direct employees
